Job Summary

Kenosha Human Development Services is seeking a highly skilled and compassionate Children's Long-Term Support Waiver Program Coordinator to join our team. As a key member of our Resource Center, you will provide critical social work and case management services to children with disabilities and their families.

Key Responsibilities
  • Provide social services and case management to an assigned caseload under the supervision of the CLTS Program Manager.
  • Maintain in-depth knowledge of the special needs, problems, and goals of clients assigned.
  • Stay up-to-date on policies and procedures for funding sources available to caseload.
  • Conduct assessments, develop individualized service plans, and monitor progress according to established guidelines.
  • Collaborate with other agencies and their personnel to ensure the best services and outcomes for clients.
  • Ensure client participation, understanding, and support of the service plan.
  • Participate in agency staffings and attend trainings as assigned.
  • Prepare correspondence, reports, and maintain records as required by agency policy.
  • Adhere to accepted standards of ethics for social workers and case managers.
Requirements
  • Bachelor's degree in human services or a related field.
  • Effective communication skills, both written and verbal, in the English language.
  • Reliable transportation and a valid driver's license with a good driving record.
  • Ability to maintain records and document case notes daily.
  • Capacity to give and accept feedback as part of a team.
  • Ability to traverse within the workspace and community frequently throughout the day.
  • Willingness to learn and work with treatment philosophies.
  • Bilingual in Spanish is a plus.
  • Experience working with children with disabilities and their families is a plus.
