Job Overview

POSITION SUMMARY:

The primary responsibility of this role is to evaluate the requirements of clients diagnosed with HIV/AIDS and to facilitate access to essential health care, psychosocial support, and additional services aimed at enhancing the overall well-being of these individuals.

KEY RESPONSIBILITIES:

  • Develops, oversees, and assesses a tailored service plan for each client.
  • Acts as an advocate, assisting clients in achieving the objectives outlined in their personalized service plans within designated timeframes.
  • Maintains collaborative relationships with social workers from local hospitals and clinics.
  • Ensures timely follow-up on referrals to community resources.
  • Engages in medical and educational programs related to health issues.
  • Integrates educational resources into the care provided to clients.
  • Works closely with clients to promote adherence to medical advice through education and collaboration with the healthcare team.
  • Completes documentation accurately and maintains thorough progress notes.
  • Delivers a minimum number of billable service units each day.
  • Ensures that client records are comprehensive, clear, and current, achieving a quarterly chart review score of 90% or higher.
  • Submits all required documentation promptly (including log sheets, case management forms, and Medicaid documentation).
  • Follows office protocols, including attendance and communication guidelines.
  • Interacts with clients, healthcare providers, and colleagues in a professional and respectful manner.
  • Completes 40 hours of continuing education and training annually.
  • Performs additional duties as assigned by the supervisor.

QUALIFICATIONS:

  • A Bachelor’s degree is required; a Master’s degree in a related field is preferred, along with at least one year of direct service experience with the HIV population.
  • Strong interviewing and assessment skills, with the ability to work effectively with diverse clients.
  • Ability to manage workload efficiently while paying attention to detail.
  • A valid driver’s license, reliable transportation, and proof of insurance are necessary for community travel.

  • Engages in continuous learning to enhance clinical and administrative skills.

  • Maintains consistent communication with the Clinic Care Manager and other healthcare providers regarding operational matters.

SUPERVISORY ROLE:

  • This position does not have supervisory responsibilities.

LANGUAGE PROFICIENCY:

  • Fluency in reading, writing, and speaking is required.

MATHEMATICAL ABILITY:

  • Competence in working with mathematical concepts such as ratios and percentages, and applying these to real-world scenarios.

REASONING SKILLS:

  • Ability to identify problems, gather data, and draw valid conclusions.
