HIV Outreach Coordinator

POSITION SUMMARY:

The Outreach Specialist plays a crucial role in connecting individuals affected by HIV/AIDS to essential healthcare services. This position is dedicated to providing support, education, and resources to enhance the continuum of care for those living with HIV/AIDS, particularly focusing on re-engaging individuals who have previously disengaged from treatment.

KEY RESPONSIBILITIES:

• Conduct HIV pre/post counseling and testing, along with STI screenings.
• Facilitate referrals to Treatment Adherence and Partner services for newly diagnosed or returning individuals.
• Collaborate with team members to organize and participate in community outreach events.
• Identify and establish HIV testing and condom distribution locations.
• Regularly inventory and monitor outreach and testing supplies.
• Provide outreach services, including case finding and client recruitment.
• Maintain up-to-date knowledge of community service providers to assist individuals who are HIV positive but not currently receiving care.
• Participate in multidisciplinary team meetings and community discussions.
• Develop and maintain relationships with social workers in local hospitals and clinics.
• Effectively follow up on referrals to community resources.
• Work closely with clients to promote adherence through education and collaboration with healthcare teams.
• Ensure accurate completion and timely submission of required documentation.
• Perform data entry of patient information into relevant systems to meet reporting requirements.
• Assist clients in accessing services and attend appointments as necessary.
• Engage in the search for and development of new outreach and testing sites.
• Participate in mandatory training sessions as required.
• Collaborate with the community to build and sustain networks related to HIV and PrEP.
• Engage in outreach activities at various community events where priority populations gather.

ADDITIONAL RESPONSIBILITIES:

• Adhere to office protocols, including attendance and communication standards.
• Possess a valid driver's license and reliable transportation for community travel.
• Maintain a professional appearance in accordance with the organization's dress code.
• Complete annual training and education requirements.
• Ensure respectful and dignified treatment of clients and their families while maintaining confidentiality.
• Wear identification while on duty.

EDUCATION AND EXPERIENCE:

• High school diploma with at least one year of experience in a public-facing role.
• Minimum of one year of direct experience in HIV services or outreach.

LANGUAGE AND COMMUNICATION SKILLS:

• Fluency in reading, writing, and speaking English.

WORK ENVIRONMENT:

• This role may involve moderate noise levels and requires compliance with the Americans with Disabilities Act.
• Physical demands include sitting, standing, walking, and occasional lifting of up to 15 lbs.
• Evening and weekend work may be necessary based on outreach activities.
