POSITION SUMMARY:

The Outreach Specialist is responsible for providing essential counseling, testing, and referral services aimed at integrating individuals with HIV/AIDS into ongoing care. This role also emphasizes the re-engagement of individuals living with HIV or AIDS who have previously disengaged from care, by educating them on the significance of continuous HIV treatment and support.

KEY RESPONSIBILITIES:

  • Conduct HIV pre/post counseling and testing services, including STI screenings.
  • Facilitate referrals to Treatment Adherence and Partner services for newly diagnosed or returning individuals.
  • Collaborate with team members to plan and participate in community outreach events.
  • Identify and establish HIV testing and condom distribution locations.
  • Regularly inventory and monitor outreach and testing supplies.
  • Deliver outreach services, focusing on case finding and client recruitment.
  • Maintain up-to-date knowledge of community service providers to identify individuals who are HIV positive but not currently receiving care.
  • Participate in multidisciplinary team meetings and community gatherings.
  • Foster relationships with social workers in local hospitals and clinics.
  • Access and follow up on referrals to community resources as needed.
  • Work closely with clients to promote adherence through education and collaboration with the medical team.
  • Ensure accurate completion and timely submission of required paperwork.
  • Input patient information into Evaluation Web and ensure timely reporting to relevant health authorities.
  • Assist clients in accessing services and accompany them to appointments when necessary.
  • Engage in ongoing training as required.
  • Develop and maintain networks related to HIV and PrEP within the community.
  • Participate in outreach activities at events where priority populations gather.

ADDITIONAL RESPONSIBILITIES:

  • Adhere to office protocols, including attendance and communication standards.
  • Maintain a valid driver's license and reliable transportation for community travel.
  • Follow the established dress code and maintain a professional appearance.
  • Complete required annual training and education.
  • Uphold client confidentiality and treat all individuals with respect and dignity.
  • Wear identification while on duty and communicate effectively with all staff members.
  • Represent the organization in a positive and professional manner.
  • Comply with all organizational policies regarding ethical practices.
  • Exemplify and promote excellent customer service.

SKILLS AND QUALIFICATIONS:

  • In-depth knowledge of HIV treatment and care protocols.
  • Ability to adapt to organizational changes and demonstrate creativity.
  • Engagement in professional development activities to enhance skills.
  • Strong organizational skills with attention to detail.
  • Flexibility in work hours and willingness to attend client appointments as needed.
  • Proficient in maintaining confidentiality and building rapport with clients.
  • Effective interviewing and assessment skills across diverse populations.
  • Self-motivated with strong problem-solving capabilities.
  • Professional interaction with clients, providers, and colleagues.

EDUCATION AND EXPERIENCE:

  • High school diploma with at least one year of experience in a public-facing role.
  • Minimum of one year of direct experience in HIV services or outreach.

LANGUAGE AND COMMUNICATION:

  • Fluency in reading, writing, and speaking English.

WORK ENVIRONMENT:

  • Work may involve moderate noise levels and various physical activities.
  • Reasonable accommodations will be made for individuals with disabilities.

PHYSICAL DEMANDS:

  • Position may require sitting, standing, walking, and lifting up to 15 lbs.
  • Travel within the community may be necessary during business hours.
