Position Overview

ROLE SUMMARY:

The Outreach Specialist plays a crucial role in providing support, education, and resources to individuals affected by HIV/AIDS. This position is dedicated to facilitating access to care for those living with HIV/AIDS and re-engaging individuals who have previously disengaged from treatment.

KEY RESPONSIBILITIES:

  • Deliver pre/post counseling and testing services for HIV, along with STI screenings.
  • Connect newly diagnosed or returning clients to Treatment Adherence and Partner services.
  • Collaborate with team members to organize and participate in community outreach events.
  • Identify and establish HIV testing and condom distribution locations.
  • Regularly assess and manage testing and outreach supplies.
  • Conduct outreach initiatives, including case finding and client recruitment.
  • Maintain active knowledge of community service providers to locate individuals who are HIV positive but not currently receiving care.
  • Participate in multidisciplinary team meetings and community discussions.
  • Build and sustain relationships with local hospital and clinic social workers.
  • Follow up on referrals to community resources as needed.
  • Support clients in adhering to treatment through education and collaboration with healthcare teams.
  • Ensure accurate completion and timely submission of necessary documentation.
  • Input patient data into Evaluation Web and ensure compliance with reporting requirements.
  • Assist clients in accessing services and accompany them to appointments when necessary.
  • Engage in the search for and development of new outreach and testing sites.
  • Attend required training sessions, both on-site and off-site.
  • Work collaboratively with the community to foster and maintain networks related to HIV and PrEP.
  • Participate in outreach activities at events where priority populations gather.

ADDITIONAL DUTIES:

  • Adhere to office protocols, including attendance and communication standards.
  • Possess a valid driver's license and reliable transportation for community travel.
  • Maintain a professional appearance in accordance with the organization's dress code.
  • Complete annual training and educational requirements.
  • Ensure respectful and dignified treatment of clients and their families while maintaining confidentiality.
  • Wear identification while on duty.
  • Communicate effectively and maintain positive relationships with colleagues.
  • Represent the organization in a professional manner.
  • Comply with all ethical business practices and organizational policies.
  • Exemplify and promote excellent customer service.

SKILLS AND QUALIFICATIONS:

  • In-depth knowledge of HIV treatment and care protocols.
  • Ability to adapt to organizational changes and demonstrate creativity.
  • Commitment to ongoing professional development and learning.
  • Strong organizational skills with attention to detail.
  • Flexibility to work varied hours and attend client appointments as needed.
  • Ability to maintain confidentiality in accordance with program guidelines.
  • Proficient in building rapport with clients from diverse backgrounds.
  • Strong interviewing and assessment capabilities.
  • Self-motivated with effective problem-solving skills.
  • Professional interaction with clients, providers, and colleagues.

EDUCATION AND EXPERIENCE:

  • High school diploma with at least one year of experience in a public-facing role.
  • Minimum of one year of direct experience in HIV services or outreach.

LANGUAGE AND COMMUNICATION:

  • Fluency in reading, writing, and speaking English.

WORK ENVIRONMENT:

  • This role may involve moderate noise levels and a variety of physical activities, including sitting, standing, and walking.

  • Evening and weekend work may be required based on outreach activities.
