Position Overview

ROLE SUMMARY:

The Outreach Specialist is responsible for providing support, testing, and referral services aimed at integrating individuals affected by HIV/AIDS into ongoing care. This role emphasizes the re-engagement of individuals living with HIV or AIDS who have previously disengaged from care, by offering education on the significance of consistent HIV treatment and management.

KEY RESPONSIBILITIES:

  • Deliver pre/post HIV counseling and testing services along with STI screenings.
  • Facilitate referrals to Treatment Adherence/Partner services for newly diagnosed or returning clients.
  • Collaborate with colleagues to plan and participate in community outreach events.
  • Identify locations for HIV testing and condom distribution.
  • Monitor and manage outreach and testing supplies on a weekly basis.
  • Conduct outreach activities, including case finding and client recruitment.
  • Maintain awareness of local service providers to connect with individuals who are HIV positive but not currently receiving care.
  • Participate in multidisciplinary team meetings and community discussions.
  • Build and sustain relationships with social workers at local hospitals and clinics.
  • Access and follow up on community resource referrals effectively.
  • Engage closely with clients to promote adherence through education and collaboration with the medical team.
  • Ensure accurate completion and timely submission of necessary paperwork.
  • Input patient data into Evaluation Web, ensuring compliance with reporting requirements.
  • Assist clients in accessing services and accompany them to appointments when necessary.
  • Perform additional tasks as assigned by supervisors.
  • Explore and establish new outreach and testing locations.
  • Attend required training sessions, both on-site and off-site.
  • Work collaboratively with the community to foster and maintain networks related to HIV and PrEP.
  • Engage in outreach activities at events where priority populations gather.

ADDITIONAL DUTIES:

  • Follow office protocols regarding attendance, communication, and participation in meetings.
  • Possess a valid driver's license, reliable transportation, and current car insurance for community travel.
  • Adhere to the organization's dress code and maintain a professional appearance.
  • Complete mandatory annual training requirements.
  • Ensure respectful and dignified treatment of clients and their families while maintaining confidentiality.
  • Wear identification while on duty.
  • Communicate effectively and maintain positive relationships with all staff members.
  • Represent the organization in a professional manner.
  • Comply with ethical business practices and organizational policies.
  • Exemplify customer service excellence.

SKILLS AND QUALIFICATIONS:

  • Knowledgeable about HIV treatment and care.
  • Support organizational initiatives and demonstrate adaptability during changes.
  • Pursue opportunities for professional development and learning.
  • Ability to manage work tasks efficiently while paying attention to detail.
  • Willingness to work flexible hours and attend client appointments as needed.
  • Understand and uphold confidentiality guidelines.

  • Possess strong interviewing and assessment skills, with the ability to work with diverse populations.
  • Demonstrate self-discipline and motivation, along with problem-solving capabilities.
  • Interact professionally and courteously with clients, providers, and colleagues.

EDUCATION AND EXPERIENCE:

  • High school diploma with at least one year of experience in a public-facing role; strong telephone etiquette and ability to engage effectively with patients.
  • Minimum one year of direct experience in HIV services or outreach.

LANGUAGE AND COMMUNICATION SKILLS:

  • Fluency in reading, writing, and speaking the English language.
