Job Overview

POSITION SUMMARY:

The Outreach Specialist plays a crucial role in connecting individuals affected by HIV/AIDS to essential care services. This position emphasizes the importance of re-engaging individuals who have previously disengaged from care, providing them with vital education on the significance of ongoing HIV treatment and support.

KEY RESPONSIBILITIES:

  • Conduct HIV pre/post counseling and testing, along with STI screenings.
  • Facilitate referrals to Treatment Adherence and Partner services for newly diagnosed or returning clients.
  • Collaborate with team members to plan and engage in community outreach events.
  • Identify and establish HIV testing and condom distribution locations.
  • Monitor and manage outreach and testing supplies on a weekly basis.
  • Implement outreach strategies to locate and recruit clients.
  • Maintain a strong network with local service providers to reach individuals who are HIV positive but not currently receiving care.
  • Participate in multidisciplinary team meetings and community discussions.
  • Foster relationships with social workers in local hospitals and clinics.
  • Ensure appropriate follow-up on referrals to community resources.
  • Support clients in adhering to treatment through education and collaboration with healthcare teams.
  • Accurately complete necessary documentation and submit reports in a timely manner.
  • Assist clients in accessing services and accompany them to appointments when required.
  • Perform additional duties as assigned by the supervisor.
  • Explore and develop new outreach and testing locations.
  • Attend required training sessions both on-site and off-site.
  • Engage with the community to build and sustain networks related to HIV and PrEP.
  • Participate in outreach initiatives at events such as Pride and other community gatherings.

ADDITIONAL RESPONSIBILITIES:

  • Follow office protocols, including attendance and communication standards.
  • Possess a valid driver's license and reliable transportation for community travel.
  • Adhere to the organization's dress code and maintain a professional appearance.
  • Complete annual training requirements.
  • Treat clients and their families with respect, ensuring confidentiality at all times.
  • Wear identification while on duty.
  • Communicate effectively and maintain positive relationships with colleagues.
  • Represent the organization in a professional manner.
  • Comply with all ethical business practices and organizational policies.
  • Exemplify customer service excellence in all interactions.

SKILLS AND QUALIFICATIONS:

  • In-depth knowledge of HIV treatment and care.

EDUCATION AND EXPERIENCE:

  • High school diploma with at least one year of experience in a public-facing role.
  • Minimum of one year of direct experience in HIV services or outreach.

LANGUAGE AND COMMUNICATION:

  • Fluency in reading, writing, and speaking.

WORK ENVIRONMENT:

  • Work is performed in various settings, including community outreach locations.
