Program: Supportive Services for Veteran Families (SSVF)

Position Title: SSVF Case Manager

Location: Springfield, IL

Job Summary: The SSVF Case Manager is responsible for delivering essential support services to Veterans and their families, utilizing a strength-based case management approach. This role aims to empower individuals to overcome crises and vulnerabilities, ultimately leading them towards sustainable housing solutions.

Reporting Structure: Reports to the SSVF Case Manager Supervisor.

Supervisory Responsibilities: None.

Qualifications:

Candidates should demonstrate a strong commitment to the mission of The Salvation Army and possess the ability to communicate this effectively. A successful completion of a background check and a valid driver's license is required.

Education: A Bachelor's degree in social work, public health, or human services is preferred, along with experience working with homeless populations or Veterans. Familiarity with VA programs is advantageous.

Skills:

  • Proficient in managing projects and engaging with diverse populations.
  • Self-motivated and capable of working independently.
  • Experience with Veterans, including knowledge of substance abuse and PTSD, is beneficial.
  • Prior training in Critical Time Intervention, harm reduction, and Trauma-Informed Care is preferred.
  • Strong written and verbal communication skills.
  • Ability to manage budgets and resources effectively.
  • Proficient in computer applications, including MS Office and social media.
  • Willingness to work flexible hours, including evenings and weekends as needed.

Key Responsibilities:

  • Foster a positive work environment through effective communication and conflict resolution.
  • Ensure compliance with safety and harm prevention standards.
  • Develop individualized case plans with participants, establishing both short- and long-term objectives.
  • Conduct regular meetings and home visits to assess progress and adjust goals.
  • Connect participants with community resources and services as necessary.
  • Facilitate educational workshops on life skills and financial management.
  • Provide referrals for those not eligible for SSVF services.
  • Maintain comprehensive case files and documentation in the Homeless Management Information System.
  • Uphold client confidentiality and manage demographic data accurately.
  • Submit monthly service statistics and participate in quality assurance initiatives.
  • Perform additional duties as assigned to support the program's mission.

Travel Requirements: Travel may be necessary for this position.

Physical Demands/Work Environment:

This role involves standard office tasks, including communication, documentation, and occasional lifting of up to 50 pounds. The work environment is primarily indoors with a low to moderate noise level.
