Corrections Transition Programs (CTP): The CTP represents a comprehensive statewide network of clinical reentry management services aimed at enhancing the prospects for successful reintegration into society. We offer specialized case management services designed to assist individuals in preparing for their return to families and communities following incarceration. Our support extends both within correctional facilities and into the community, providing essential reentry assistance that includes behavioral health referrals, public benefits enrollment, employment and housing resources, and obtaining vital records.
Position Summary: We are currently seeking a full-time Case Manager.
Compensation: Starting salary is $41,600, contingent upon experience and educational background.
Key Responsibilities:
- Deliver direct services and coordinate additional resources to help clients achieve their established service plan objectives.
- Conduct screenings and assessments to determine client eligibility for agency services.
- Adhere to the established service hour protocols of the agency.
- Encourage client engagement in drug treatment and community activities.
- Provide counseling and support to family members to help them understand and assist the client.
- Facilitate individual and group orientation or educational sessions.
- Administer client drug tests as required.
- Represent clients and the agency in staffing meetings, court hearings, and provider networks.
- Offer referrals for identified client needs and provide crisis intervention when necessary.
- Document assessments, service plans, reports, progress notes, discharge summaries, and other client-related activities accurately.
- Participate in in-service training, conferences, and educational sessions as required.
- Maintain all necessary documentation related to clients, including letters and status reports.
Qualifications:
- Bachelor's degree in human services, social work, psychology, or a related field is preferred.
- One or more years of experience in human services or a comparable combination of training and experience.
- Understanding of human behavior, including assessment and identification of substance abuse disorders, particularly for special populations.
- Familiarity with the Code of Ethics for substance abuse professionals.
- Knowledge of federal drug and alcohol confidentiality laws (42 C.F.R. Part 2) and the Health Insurance Portability and Accountability Act.
- Ability to obtain CARS or CADC certification through the State Certification Board (IAODAPCA) within the first two years of employment, if required by the program.
- Awareness of the criminal justice system.
- Proficiency in computer skills, including Microsoft Office applications and internet navigation.

Work Environment: The agency currently offers hybrid work schedules that combine in-office and remote work. Employees are expected to report to the work site three days per week, with the option to work remotely for up to two days.
