Our integrated care teams are devoted to addressing the holistic needs of individuals, and we are currently looking for Case Managers who are enthusiastic about uplifting veterans and their families during challenging times.

As a Case Manager, you will play a vital role in our Supportive Services for Veteran Families program, focusing on enhancing the well-being and stability of veterans and their families who are experiencing homelessness, housing instability, or residing in community-based subsidized housing.

Our programs are designed to assist individuals in achieving their goals and wellness through a Housing First, client-centered, and trauma-informed approach.

Key Responsibilities:

- Maintain a caseload of veteran clients.
- Develop housing plans and budgets with households, assisting veterans in accessing resources to secure or maintain permanent housing.
- Support efforts to increase household income and prepare veteran families for independent housing and financial stability alongside our integrated services team.
- Conduct assessments and coordinate services/referrals with VA Medical Centers and other community organizations.
- Assist households in applying for housing and subsidy programs, as well as state and federal benefits.
- Maintain regular communication with veteran households and community partners.
- Identify and mitigate barriers to permanent housing and conduct housing inspections for potential units.
- Provide education on tenant rights and responsibilities.
- Participate in supervision, team meetings, program audits, training, and professional development.
- Uphold professional standards and ensure all services are documented accurately.

Qualifications:

- BA/AA degree preferred; alternatively, a minimum of 2 years of relevant experience in a case management role may be considered.
- Familiarity with veteran issues, homelessness, and state and federal resources is highly desirable.
- Proficiency in electronic health records, Microsoft Word, and Outlook is preferred.
