Position Overview
The Registered Nurse (RN) plays a crucial role within the interdisciplinary team, dedicated to addressing the comprehensive needs—physical, medical, psychosocial, emotional, and spiritual—of patients and families navigating terminal illness and bereavement.

Core Responsibilities
The RN is responsible for coordinating care, continuously assessing the needs of patients and families, and implementing the interdisciplinary care plan.

  1. Case Management and Oversight
    The RN adheres to established policies and standards, ensuring compliance with the scope of the RN license and hospice eligibility requirements.
  2. The RN communicates any requests for practice outside the licensed scope to the appropriate supervisor.
  3. The RN supervises nursing tasks delegated to Hospice Aides, Licensed Practical/Vocational Nurses, and Volunteers, ensuring adherence to the care plan.
  4. Conducts thorough initial and ongoing assessments, addressing physical, psychosocial, spiritual, and bereavement needs to alleviate end-of-life symptoms.
  5. Identifies and utilizes necessary resources and referrals, including Physical Therapy, Occupational Therapy, Home Health, and more.
  6. Documents care events promptly and accurately at the care location.
  7. Provides education and training to patients, families, and caregivers regarding patient care, including pain management strategies.
  8. Participates in patient care meetings and weekly team discussions.
  9. Delivers effective communication through various reporting methods.
  10. Engages in bereavement support activities.
  11. Participates in on-call rotations and provides care during non-business hours as needed.
  12. Collaborates with the Interdisciplinary Group (IDG) to develop and follow individualized care plans.
  13. Obtains physician orders as necessary and delivers specialized nursing services.
  14. Evaluates the effectiveness of care services provided.
  15. Delegates nursing tasks appropriately while ensuring patient safety and competency of aides.
  16. Monitors compliance with care plans and participates in quality improvement initiatives.
