Job Summary

The Elizabeth Hospice is seeking a skilled Licensed Vocational Nurse to join our team as a Hospice Care Specialist. In this role, you will provide professional nursing care to patients in their homes or facilities, working under the supervision of a Registered Nurse. As a key member of our interdisciplinary team, you will be responsible for delivering high-quality patient care, collaborating with colleagues to ensure seamless service delivery, and adhering to our organization's standards and policies.

Key Responsibilities
  • Assist in analyzing patient situations, anticipating potential issues, and taking appropriate action under the guidance of the RN.
  • Evaluate patient responses to care, ensuring timely and safe outcomes.
  • Perform licensed nurse skills, adhering to our policies, procedures, and standards of care.
  • Collaborate with the RN to develop and implement patient care plans.
  • Deliver patient education and support under the supervision of the RN.
  • Document patient care, including telephone interactions with colleagues, in a timely and accurate manner.
  • Enter and retrieve data from electronic files, maintaining confidentiality and adhering to data security protocols.
  • Orient, instruct, and train assigned personnel, ensuring they are equipped to provide high-quality patient care.
  • Perform phlebotomy as needed, adhering to established protocols.
  • Support the organization in completing assigned audits and other office duties as required.
Requirements
  • Vocational/Specialized Training.
  • Minimum 12 months of experience as an LVN, with a preference for 1-2 years in an acute care setting, home health, or hospice.
  • Current California LVN license.
  • Ability to communicate effectively, maintain patient charts, and schedule appointments.
  • Proficiency in using calculators, computers, and copy machines.
  • Phlebotomy certification preferred.
  • Pediatric experience preferred.
  • Bi-lingual (Spanish) preferred.
  • Current California driver's license and proof of insurance required.
