Key Responsibilities- Assist in analyzing patient situations, anticipating potential issues, and taking appropriate action under the guidance of the RN.
- Evaluate patient responses to care, ensuring timely and safe outcomes.
- Perform licensed nurse skills, adhering to our policies, procedures, and standards of care.
- Collaborate with the RN to develop and implement patient care plans.
- Deliver patient education and support under the supervision of the RN.
- Document patient care, including telephone interactions with colleagues, in a timely and accurate manner.
- Enter and retrieve data from electronic files, maintaining confidentiality and adhering to data security protocols.
- Orient, instruct, and train assigned personnel, ensuring they are equipped to provide high-quality patient care.
- Perform phlebotomy as needed, adhering to established protocols.
- Support the organization in completing assigned audits and other office duties as required.
- Vocational/Specialized Training.
- Minimum 12 months of experience as an LVN, with a preference for 1-2 years in an acute care setting, home health, or hospice.
- Current California LVN license.
- Ability to communicate effectively, maintain patient charts, and schedule appointments.
- Proficiency in using calculators, computers, and copy machines.
- Phlebotomy certification preferred.
- Pediatric experience preferred.
- Bi-lingual (Spanish) preferred.
- Current California driver's license and proof of insurance required.
