Position Overview:
We are seeking a Patient Care Coordinator LPN/LVN to oversee the daily operations of our hospice site. In this role, you will report directly to the Executive Director or Administrator and will be responsible for coordinating the services provided by our multidisciplinary team.
Key Responsibilities:
- Schedule visits requested by Registered Nurses, Social Workers, and Chaplains.
- Ensure timely completion of visits by staff members.
- Act as the scribe and facilitator during interdisciplinary team meetings.
- Collect necessary information to complete patient referrals.
- Verify eligibility for Medicare patients and confirm commercial insurance coverage.
Compensation:
The salary for this position ranges from $25.00 to $33.00 per hour, influenced by various factors such as location, experience, education, skills, licensure, certifications, and departmental equity. Gentiva offers a comprehensive benefits and rewards package, of which base pay is just one component.
Qualifications:
- Graduation from an accredited Practical Nursing program (must be licensed in the state of employment).
- A minimum of two years of direct patient care experience in a clinical environment, preferably in hospice or home health.
- Three to five years of experience in an administrative capacity.
- Proficient in operating nursing assessment tools and office equipment, including computers and tablets.
- Demonstrated sensitivity to the needs of terminally ill patients and their families.
- Understanding of hospice care principles and the challenges faced by terminally ill patients and their families.
