Key Responsibilities- Assist in analyzing patient situations, anticipating problem areas, and taking appropriate action.
- Evaluate patient responses to care, ensuring timely and safe outcomes.
- Support field staff with electronic medical record entries and troubleshooting.
- Perform licensed nurse skills, adhering to policies, procedures, and standards of care.
- Implement and document teaching plans, tailored to patient and family needs.
- Document all care provided, including telephone interactions with staff members.
- Enter and retrieve data from computer files, both on-site and remotely.
- Orient, instruct, and train assigned personnel.
- Support agency operations, including audits and office duties.
- Participate in cross-training in Clinical, Triage, Admissions, Access Center, and Program Representative roles.
- Vocational or specialized training.
- Current California LVN license.
- Ability to answer phones, maintain patient charts, and schedule appointments.
- Intermediate proficiency in Office applications and EMR system.
- 3 years of clinical experience, with hospice experience required.
- Licensed driver with an insured automobile.
