Key Responsibilities
  • Assist in analyzing patient situations, anticipating problem areas, and taking appropriate action.
  • Evaluate patient responses to care, ensuring timely and safe outcomes.
  • Support field staff with electronic medical record entries and troubleshooting.
  • Perform licensed nurse skills, adhering to policies, procedures, and standards of care.
  • Implement and document teaching plans, tailored to patient and family needs.
  • Document all care provided, including telephone interactions with staff members.
  • Enter and retrieve data from computer files, both on-site and remotely.
  • Orient, instruct, and train assigned personnel.
  • Support agency operations, including audits and office duties.
  • Participate in cross-training in Clinical, Triage, Admissions, Access Center, and Program Representative roles.
Requirements
  • Vocational or specialized training.
  • Current California LVN license.
  • Ability to answer phones, maintain patient charts, and schedule appointments.
  • Intermediate proficiency in Office applications and EMR system.
  • 3 years of clinical experience, with hospice experience required.
  • Licensed driver with an insured automobile.
