Position Overview:
We are seeking a Patient Care Coordinator LPN/LVN to oversee the daily operations of our hospice site and facilitate the coordination of services among all team members.

  • Organizing visit schedules requested by Registered Nurses, Social Workers, and Chaplains.
  • Monitoring timely completion of visits by staff.
  • Acting as the scribe and facilitator during interdisciplinary team meetings.
  • Collecting essential information to ensure complete referrals.
  • Verifying eligibility for Medicare patients and confirming commercial insurance coverage.

Compensation:
The salary range for this position is $25.00/hour - $33.00/hour, which may vary based on factors such as location, experience, education, skills, licensure, certifications, and departmental equity. Gentiva provides a comprehensive benefits package, of which base pay is just one component.

Qualifications:

  • Graduation from an accredited Practical Nursing program (must be licensed in the state of employment).
  • A minimum of two years of direct patient care experience in a clinical environment, preferably in hospice or home health.
  • Three to five years of experience in an administrative capacity.
  • Proficiency in utilizing nursing assessment tools and office technology, including computers and tablets.
  • Empathy towards the feelings of terminally ill patients and their families during and after the patient’s passing.
  • Understanding of the needs of terminally ill patients and their families, along with knowledge of hospice principles.
