RADIAS Health is seeking a dedicated full-time Client Support Specialist to become part of our Targeted Case Management team.


The Client Support Specialist delivers individualized services aimed at fostering recovery and community integration for adults experiencing serious persistent mental health challenges and co-occurring substance use issues.

Our mission is to assist these individuals in achieving and sustaining an optimal quality of life that aligns with their unique needs and preferences.

Client Support Specialists facilitate access to essential medical, social, educational, vocational, and other relevant services tailored to the client's mental health requirements.


Work Schedule:
Monday - Friday, 8 am - 4 pm or 9 am - 5 pm, totaling 40 hours per week.

This position operates on a hybrid model, allowing for both community engagement with clients and remote work opportunities.

Key Responsibilities:


1. Develop and execute treatment plans based on diagnostic and functional assessments, with approval from the clinical supervisor.

2. Collaborate with team members to ensure adherence to Minnesota Rule Chapter 9520.

3. Provide or facilitate access to necessary medical, vocational, social, educational, financial, legal, housing, and substance use services.

4. Create an Individual Community Support Plan for each client biannually.


5. Schedule in-person visits as outlined in the ICSP.


6. Make culturally appropriate referrals and assist clients in accessing required services.

7. Participate in the commitment process as required, including providing testimony in probate court and preparing necessary court documentation.

8. File all mandated reports with relevant authorities.


9. Work with team members to deliver services that enhance each client's ability to live independently.

10. Develop a personalized emergency support plan in coordination with community emergency services.


11. Assist clients with housing challenges and liaise with housing authorities and landlords.


12. Attend scheduled quarterly reviews and treatment planning sessions.


13. Collaborate with and educate family members and other stakeholders involved in the client's care.

14. Provide clients with information regarding their symptoms, treatment options, and recovery process.


15. Ensure comprehensive coverage of client needs within the team.


16. Establish and maintain a therapeutic relationship with clients.


17. Complete all documentation, reports, and charting in a timely and organized manner.


18. Engage in ongoing professional development and maintain appropriate professional conduct.


19. Transport clients to various appointments as needed.


Qualifications:

1. Bachelor's degree in behavioral sciences or a related field (or 30 semester credit hours in behavioral health sciences with relevant experience).

2. Minimum of 2,000 hours of supervised experience working with individuals diagnosed with serious mental illness and/or substance use disorders - OR -

3. A Master's or other advanced degree in behavioral health sciences or a related field - OR -

4. If no degree, a minimum of 4,000 hours of supervised experience in the same field.


Additional Requirements:

Must possess a valid driver's license, auto insurance, and access to a vehicle.
