Client Support Coordinator

Our Client Support Coordinators play a vital role in enhancing the quality of support services, ensuring client satisfaction, and promoting efficient resource utilization.
Key Responsibilities:
- Develop individualized community support plans tailored to each client's goals.
- Assist clients in accessing necessary services and creating service agreements while maintaining thorough documentation.
- Inform clients or their legal guardians about suitable service options.
- Conduct biannual one-on-one meetings with clients to assess progress and adjust support plans as needed.
- Identify potential service providers to help clients achieve their objectives.
- Evaluate and monitor the effectiveness of services outlined in the support plan.
- Ensure compliance with relevant case management regulations.
- Adhere to health care program guidelines related to waiver services, including conducting necessary assessments and utilizing state systems.

Requirements:
- A Bachelor's Degree in Human Services, Social Work, Psychology, Sociology, or a related field.
- If holding a Social Work degree, current licensure as required by the Minnesota Board of Social Work is expected.
- Alternatively, a Bachelor's degree in any other field with at least one year of experience working with individuals with mental health challenges or disabilities.
- Strong written and verbal communication skills.
- Ability to work independently and effectively in crisis situations.
- Comfortable managing high volumes of paperwork with attention to detail.
- Must pass a background check and possess a valid driver's license with a clean driving record.
- Reliable transportation is essential.
Work Environment:
The Client Support Coordinator will primarily work in community and office settings. The position typically requires full-time hours, with a Monday to Friday schedule and some flexibility based on program needs.
