Higher Ground - St Paul Medical Respite

SALARY RANGE: $22.50-$26.30/hour

POSITION SUMMARY: Catholic Charities is looking for a proactive advocate who is empathetic, trauma-informed, and culturally competent to take on the role of Client Support Specialist II at its Higher Ground - St Paul location within the Medical Respite program.

This role involves direct engagement with assigned clients to formulate the client's service plan, which includes conducting intake assessments of clients' needs, identifying available community partners and resources, and utilizing relevant Catholic Charities services. The candidate will comply with case documentation standards and work closely with clients to promote their success, monitor their progress, and modify service plans as needed.

The Client Support Specialist will act as an advocate for clients and provide education regarding services and challenges faced by the community.

This position necessitates a valid Minnesota driver's license, a dependable vehicle, and proof of insurance.

KEY RESPONSIBILITIES:

  1. Conducts initial assessments, develops, reviews, and maintains a service plan tailored to meet the client's needs, incorporating both Catholic Charities programs and other community resources.
  2. Advocates for clients in securing services and benefits, assisting with necessary documentation.
  3. Facilitates referrals to and coordinates suitable community resources for clients.
  4. Maintains thorough case documentation and records as mandated by Catholic Charities, funding sources, or regulatory bodies.
  5. Engages in community education, informing volunteers, service providers, and other stakeholders about client services and programs.
  6. Collaborates with supervisors on all aspects of client care and seeks appropriate guidance and feedback from team members.
  7. May provide transportation for clients.
  8. Consults and collaborates with community systems to ensure linkage, referral, crisis management, and follow-up, focusing on achieving client goals.
  9. Delivers assessment, education, and case management services to a diverse, often long-term group of assigned clients, which may include home visits.
  10. Advocates for clients in obtaining necessary services.
  11. Performs other duties as assigned.

MINIMUM QUALIFICATIONS:

  • Bachelor's Degree in human services or a related field preferred, with at least two (2) years of experience working with a specific client demographic.
    1. Alternatively, a minimum of three (3) years of experience working with a specific client group is acceptable in lieu of a degree.
  • Must possess a valid driver's license and current insurance coverage if transporting clients.
  • Demonstrated ability to work with diverse and/or low-income populations, including individuals experiencing homelessness, unstable housing, and/or mental health challenges.
  • Additional training, education, or licensure may be required based on program needs.
