Job Overview

The Children's Clinic is seeking a dedicated Care Coordinator to enhance our commitment to delivering exceptional healthcare services to patients of all ages. This role is vital in ensuring patients receive the necessary referrals and support for their healthcare needs.

Key Responsibilities:

  • Facilitate patient referrals to both internal and external resources, ensuring seamless access to specialty care.
  • Coordinate appointment scheduling for patients with healthcare providers, maintaining effective communication with the care team.
  • Act as a liaison among healthcare professionals, including Providers, Registered Nurses (RNs), Licensed Vocational Nurses (LVNs), and Care Team Supervisors, to guarantee comprehensive patient care.
  • Engage positively with staff, patients, families, and community members to foster a supportive environment.

Essential Duties:

  • Prepare and manage routine reports, messages, and documentation with attention to detail.
  • Communicate effectively in both written and verbal formats, adhering to proper grammar and professionalism.
  • Organize and maintain patient records according to established protocols, ensuring confidentiality and accuracy.
  • Utilize office equipment efficiently, including computers, fax machines, and photocopiers.
  • Participate in team meetings and contribute to collaborative efforts in patient care.
  • Process authorizations and referrals, ensuring timely follow-up to maintain continuity of care.

Specific Competencies:

  • Conduct follow-ups on missed appointments, particularly for patients with special needs.
  • Collaborate with external agencies, including schools and public health departments, to support patient care.
  • Assist in coordinating surgical procedures and pre-operative assessments with specialist offices.
  • Support health department initiatives by following up on referrals and assisting with patient forms.
  • Provide interpretation and cultural support for patients as needed.

Qualifications:

  • Certification as a Medical Assistant is required.
  • A High School Diploma or GED is necessary.
  • 2-4 years of experience in a medical office setting is preferred.
  • Proficiency in filing and operating office equipment is essential.
  • Strong organizational and communication skills, with a keen attention to detail.
  • Bilingual in Spanish or Khmer is highly desirable.

Work Environment:

The work environment is typically moderate in noise level, situated in an indoor office setting with shared workspace. Employees may occasionally work outdoors and will experience varying weather conditions.
