Job Overview

Position Summary

The Care Coordinator plays a pivotal role within the Advanced Medical Management organization, specifically within the Care Management Department. This position is tasked with facilitating telephonic patient support in accordance with established guidelines and in collaboration with the Care Management Team. A positive demeanor, compassion, and professionalism are essential attributes for success in this role, with the ultimate goal of delivering exceptional patient care.

The ideal candidate will demonstrate strong problem-solving capabilities and the ability to implement solutions efficiently. Daily responsibilities include making calls to both patients and healthcare providers, necessitating effective time management skills.

Key Responsibilities

  • Manage all OnBase documentation for the Care Management department.
  • Maintain an average workload of 5-20 cases daily.
  • Handle all Tracking and TPL cases effectively.
  • Assist in resolving Bed Days Audit Manager discrepancies.
  • Provide coverage for other Inpatient Coordinators during their absence.
  • Stay informed about AMM Policies, Procedures, and relevant laws and regulations.
  • Ensure patient safety and confidentiality are upheld at all times.
  • Exhibit flexibility and a willingness to assist patients as needed.
  • Adhere to the Employee Handbook policies and procedures.
  • Utilize ADP in accordance with AMM Policies and Procedures.
  • Engage in team meetings and contribute to collaborative efforts.
  • Request paid time off through ADP as necessary.
  • Initiate the Access program by preparing a list of daily calls, ensuring all call dates are current.
  • Conduct daily phone outreach as scheduled, documenting all relevant details on flow sheets and face sheets.
  • Update the Access program with information such as call dates, follow-up calls, and patient appointments.
  • Regularly check the fax machine for hospital discharge documentation.
  • Assist with Health Risk Assessments under the guidance of the Care Manager to identify and address patient needs.
  • Monitor the hospital log for High Risk patients and maintain accurate records.
  • Check voicemail throughout the day and return patient calls promptly, informing them of any necessary follow-up by nursing staff.
  • Ensure all charting is reviewed and signed off by a Nurse, with no exceptions.
  • Maintain a thorough understanding of AMM Policies and Procedures, ensuring compliance with confidentiality standards.
  • Demonstrate a positive attitude and strong interpersonal skills while providing patient support.
  • Participate actively in staff meetings to foster team collaboration.

Qualifications and Skills

  • Preferred: One year of experience in a medical office or clinic, or as a Coordinator in an IPA/Group UM or CM department. Candidates with a BA degree from an accredited institution will also be considered.
  • Strong verbal and written communication skills.
  • Commitment to maintaining patient confidentiality.
  • Ability to support departmental and organizational objectives.
  • Preferred: At least one year of experience in a medical or healthcare setting.
  • High energy, goal-oriented mindset.
  • Excellent problem-solving and time management abilities.
  • Proficient in Microsoft Word, Outlook, and Excel.
  • Exemplary work ethic and positive interpersonal skills.
  • Availability to work Monday through Friday during standard business hours.
