Position Overview

Key Responsibilities

The RN Care Coordinator plays a crucial role in evaluating, planning, facilitating, and advocating for patient care options throughout the continuum of services, from initial contact to discharge. This position is designed to address individual health requirements while ensuring quality and cost-effective outcomes. The primary focus will be on:

  • Coordinating care through effective communication and collaboration with nursing staff, physicians, and various departments within the healthcare system.
  • Engaging with insurers and post-acute service providers to streamline care pathways towards optimal patient outcomes.
  • Advocating for patients and their families by recognizing and respecting their choices, spiritual needs, cultural backgrounds, and socioeconomic barriers that may affect care transitions.
  • Maintaining patient confidentiality while striving for high levels of satisfaction.
  • Conducting utilization reviews and discharge planning in accordance with established hospital policies and regulatory standards.

Travel Requirements: Minimal travel is expected (less than 10%).

Qualifications
  • Active Registered Nurse (R.N.) license.
  • A minimum of three years of nursing experience in an acute care hospital environment.
  • Previous experience in care coordination within a clinical or insurance context is essential.
  • Consideration may be given to candidates without the required experience if operational conditions allow.
  • Certification in Basic Life Support (BLS) from AHA is required.
  • Strong customer service and presentation skills are essential, along with excellent interpersonal and written communication abilities.
  • Demonstrated analytical and problem-solving skills, with the capacity to manage multiple tasks or projects effectively.

Preferred Qualifications:

  • Bachelor of Science in Nursing (BSN) or equivalent experience is preferred.
  • Certification in Case Management (CM) is advantageous.
  • A Master's Degree in nursing with a focus on Case Management may substitute for the experience requirement.
