Key Responsibilities

The **RN Care Coordinator** plays a crucial role in evaluating, planning, and facilitating care for patients from the initial contact through to discharge. This position is essential in ensuring that patients receive comprehensive care tailored to their individual health requirements while also promoting effective and economical outcomes.

The RN Care Coordinator will:

  • Collaborate with healthcare professionals, including nursing staff and physicians, to ensure seamless care transitions.
  • Advocate for patients and their families by recognizing and addressing their preferences, cultural needs, and socioeconomic challenges.
  • Maintain patient confidentiality while striving for exceptional satisfaction levels.
  • Conduct utilization reviews and discharge planning in accordance with established policies and regulations.

**Travel Requirements:** Minimal travel (less than 10%) may be required.

Qualifications
  • Current Registered Nurse (R.N.) license.
  • A minimum of 3 years of nursing experience in an acute care environment.
  • Experience in care coordination within clinical or insurance settings is essential.
  • Basic Life Support certification (AHA CPR BLS) is required.
  • Strong interpersonal and communication skills are vital, along with the ability to manage multiple tasks effectively.

Preferred Qualifications:

  • Bachelor of Science in Nursing (BSN) or equivalent experience is preferred.
  • Certification in Case Management (CM) is advantageous.
  • A Master's Degree in nursing with a focus on Case Management may substitute for experience requirements.
