Position Title: Community Support Coordinator
Department: School & Community Youth Services

The **Community Support Coordinator** is responsible for delivering culturally and linguistically appropriate case management services within the **School and Community Youth Services** program.

Key Responsibilities:
  • Deliver comprehensive case management services for the **SCYS** program.
  • Engage with educational and community partners to ensure holistic care.
  • Organize and lead meetings, workshops, and events to foster collaboration between families and educational institutions.
Essential Duties:
  • Facilitate support groups and case management services for students, parents, and families.
  • Document and regularly update service plans.
  • Maintain consistent communication with clients.
  • Coordinate and conduct meetings, workshops, presentations, and events to strengthen partnerships between families and schools.
  • Establish a referral network to ensure a continuum of care.
  • Participate in relevant workshops and conferences as necessary.
Qualifications:
  • Bachelor's degree in Social Work (B.S.W.), Psychology, or a related discipline.
  • Experience with collaborative school-based service delivery models is preferred.
  • Familiarity with community resources in the local area is advantageous.
  • Ability to engage in moderate physical activity within the community.
  • Valid state driver's license is required.
  • Bilingual candidates (English/Spanish) are preferred.

Reporting Structure: Reports to the Program Director.
