Position Summary:

Job Title: Family Support Coordinator FLSA Status: Non-Exempt
Department: School & Community Youth Services
The Family Support Coordinator is responsible for delivering services in Montgomery County, MD.
Key Responsibilities:
- Deliver culturally and linguistically appropriate case management services for the School and Community Youth Services (SCYS) mental health and social services program in partnership with local public schools and health departments.
- Focus on enhancing self-sufficiency skills, family development, and connecting families to mental health resources, particularly targeting under-insured youth and families in need.
- Facilitate support groups to enhance the services provided.
- Collaborate with therapists, school personnel, and community partners to ensure comprehensive care while safeguarding client confidentiality.
- Provide case management and support group services to families facing economic and social challenges, including housing, healthcare, employment, food security, and education.
- Utilize the County's electronic records system to document service plans and conduct regular updates every six months. Maintain consistent communication with clients, at least twice monthly.
- Ensure timely data entry into the system within one business day of service delivery.
- Manage a caseload in accordance with contract stipulations and agency expectations as outlined by the program director.
Work closely with service providers to establish a referral system that promotes continuous care.
Conduct home visits as necessary and make referrals when appropriate.
Participate in workshops and conferences as requested by the immediate supervisor.
Qualifications:
- Bachelor's degree in Social Work (B.S.W.), Psychology, or a related field. Relevant experience serving families in need is considered (internship experience is applicable). Prior experience may substitute for required education on a year-for-year basis.
- Preferred experience in collaborative school-based service delivery models.
- Knowledge of immigrant populations and acculturation issues is advantageous.
- Familiarity with community resources in the Montgomery County area is preferred.
- A valid state driver's license with three or fewer points at the time of hire is generally required.
- Bilingual candidates (English/Spanish) are preferred but not mandatory.
- This role is partially community-based, with time allocated to an office environment.
- Ability to travel within designated areas.
- Maintain moderate physical activity, including walking, lifting, and driving.
- Reports directly to the Program Director.
Job Type: Full-time
Salary: $52,000.00 annually
