About the Role

We are seeking a compassionate and skilled Nursing Assistant to join our team at VIA Health Partners, a leading provider of end-of-life care. As a Hospice House Nursing Assistant, you will play a vital role in delivering high-quality patient care and supporting our patients and their families during a challenging time.

Key Responsibilities
  • Deliver personal care to patients, including bathing, body mechanics, nutrition, and elimination
  • Provide safety measures, such as side rails, mitts, and restraints, to ensure patient well-being
  • Perform special procedures, such as vital signs, wound care, and postmortem care
  • Participate in interdisciplinary team conferences and special case conferences
  • Document patient care and progress accurately and timely
Requirements
  • High School diploma or equivalent required; Certified Nursing Assistant (CNA) certification preferred
  • Minimum 1 year experience in home care, geriatrics, or hospice care
  • Ability to work independently and as part of a team
  • Excellent communication and interpersonal skills
