Key Responsibilities- Coordinate patient care plans, ensuring comprehensive and individualized support for patients and their families.
- Conduct thorough assessments, identifying physical, emotional, and spiritual needs, and develop effective interventions to address these needs.
- Collaborate with the interdisciplinary team to ensure seamless care coordination and communication.
- Provide education and support to patients, families, and caregivers, promoting self-advocacy and empowerment.
- Maintain accurate and detailed patient records, adhering to organizational guidelines and regulations.
- Graduation from an accredited school of nursing; Bachelor of Science in Nursing (BSN) preferred.
- Previous nursing experience in a hospital, home health agency, or hospice setting preferred; new graduates also eligible.
- Licensed as a Registered Nurse (RN) in Arkansas.
- Ability to work flexible hours and occasional overtime required.
- Knowledge of palliative care principles and symptom management.
- Ability to communicate effectively with patients, families, and healthcare teams.
- Strong organizational and time management skills, with attention to detail and ability to prioritize tasks.
- Ability to maintain confidentiality and handle sensitive information with discretion.
- Strong teamwork and collaboration skills, with a patient-centered approach.
This is a safety-sensitive position, requiring regular handling of confidential information and performing life-threatening procedures. Any lapse of attention could have a significant impact on Arkansas Hospice and/or its patients.
