Job Summary

We are seeking an experienced Hospice Nurse to join our team at (434300) Medi Home Health & Hospice. As a Hospice Nurse, you will play a vital role in providing high-quality patient care and support to individuals in need of end-of-life care.

Key Responsibilities
  • Develop and implement individualized care plans for patients, taking into account their unique needs and goals.
  • Provide skilled nursing care to patients, including administering medications, wound care, and other treatments as needed.
  • Collaborate with the interdisciplinary team to ensure comprehensive care and support for patients and their families.
  • Conduct regular assessments and evaluations to identify changes in patient condition and adjust care plans accordingly.
  • Communicate effectively with patients, families, and healthcare team members to ensure seamless care coordination.
Requirements
  • Current RN licensure in the state of VA.
  • Minimum 1 year of experience in hospice care or a related field.
  • Strong clinical skills and ability to work independently.
  • Excellent communication and interpersonal skills.
  • Ability to work in a fast-paced environment and prioritize tasks effectively.
