Job Summary

We are seeking a dedicated and compassionate Hospice Nursing Assistant to join our team. In this role, you will provide personalized care to patients in their homes, focusing on their physical, emotional, and spiritual well-being. As a member of our interdisciplinary team, you will work closely with nurses, social workers, and other healthcare professionals to deliver comprehensive care.

Responsibilities
  • Deliver patient care, including personal care, wound management, and vital sign monitoring
  • Participate in interdisciplinary team conferences to discuss patient care plans and goals
  • Assist with daily living activities, such as bathing, dressing, and feeding
  • Provide emotional support and companionship to patients and their families
  • Document patient care and progress in accordance with HPCCR guidelines
Requirements
  • High school diploma or equivalent required; training/certification in related field preferred
  • Minimum 1 year experience in home care, geriatrics, or hospice care
  • Current listing status as a Certified Nursing Assistant (CNA) in good standing with the NC Department of Health and Human Services
