The Patient Care Coordinator collaborates with the healthcare team to provide continuous support and communication for patients with chronic kidney disease (CKD) and End-Stage Renal Disease (ESRD). This role serves as a central point of contact, coordinating resources, identifying care gaps, and ensuring proactive follow-up.
Daily Activities
- Conducts outreach calls to patients to assess their clinical needs and connect them with suitable resources.
- Follows up with patients to confirm their needs are met and schedules future check-ins.
- Works closely with clinical teams and providers to ensure a smooth continuum of care.
- Monitors patient hospitalizations and collaborates with care team members to gather relevant patient information.
- Distributes educational materials to patients and communicates with primary care physicians and specialists regarding new appointments.
- Engages effectively with all levels of the clinical team to manage care transitions and address any care gaps.
- Minimum of 2 years of relevant education, experience, or certification.
- Current BLS or CPR Certification is required.
- Reliable transportation for commuting to patient locations.
- Ability to provide in-person patient care, which may involve physical activities.
- Internet connectivity meeting specified requirements.
- Active Community Health Worker (CHW) Certification.
- Experience in customer service.
- Proficiency in MS Office applications.
- Strong verbal and written communication skills.
- Ability to handle confidential information with discretion.
- Clear and respectful communication style.
