Key Responsibilities

The Patient Care Coordinator collaborates with the healthcare team to provide continuous support and communication for patients with chronic kidney disease (CKD) and End-Stage Renal Disease (ESRD). This role serves as a central point of contact, coordinating resources, identifying care gaps, and ensuring proactive follow-up.

Daily Activities
  • Conducts outreach calls to patients to assess their clinical needs and connect them with suitable resources.
  • Follows up with patients to confirm their needs are met and schedules future check-ins.
  • Works closely with clinical teams and providers to ensure a smooth continuum of care.
  • Monitors patient hospitalizations and collaborates with care team members to gather relevant patient information.
  • Distributes educational materials to patients and communicates with primary care physicians and specialists regarding new appointments.
  • Engages effectively with all levels of the clinical team to manage care transitions and address any care gaps.
Qualifications
  • Minimum of 2 years of relevant education, experience, or certification.
  • Current BLS or CPR Certification is required.
  • Reliable transportation for commuting to patient locations.
  • Ability to provide in-person patient care, which may involve physical activities.
  • Internet connectivity meeting specified requirements.
Preferred Skills
  • Active Community Health Worker (CHW) Certification.
  • Experience in customer service.
  • Proficiency in MS Office applications.
