POSITION OVERVIEW

  • The Care Manager is responsible for enhancing the psychosocial well-being of residents through comprehensive assessments, tailored treatment strategies, and therapeutic interventions. This role includes facilitating benefit management and serving as a crucial link between residents, their families, professionals, and community resources to promote resident autonomy. The Care Manager advocates for the rights and needs of individuals with disabilities, providing guidance and support in conflict resolution and problem-solving.

KEY RESPONSIBILITIES

  • ASSESSMENT OF RESIDENT PSYCHOSOCIAL NEEDS
  • Conduct thorough clinical evaluations, including psychosocial assessments, mood interviews, depression screenings, and risk assessments to identify residents' needs and capabilities.

CREATION AND IMPLEMENTATION OF THERAPEUTIC PLANS

  • Develop individualized and group counseling sessions, empower residents through life skills training, and implement behavior management strategies while providing crisis intervention and support in goal-setting.

STAFF TRAINING AND DEVELOPMENT

  • Deliver formal and informal training sessions, offer ongoing staff support, and create educational materials to enhance staff knowledge and skills.
  • COORDINATION OF RESIDENT SERVICES
  • Establish collaborative relationships with community resources to ensure residents receive appropriate services tailored to their unique needs, including mental health support, educational opportunities, and financial management.

ADVOCACY FOR RESIDENT RIGHTS

  • Actively advocate for residents within the organization and the broader community, facilitating resident and family council meetings to ensure their voices are heard.

DISCHARGE PLANNING COORDINATION

  • Oversee the Community Transition Program for residents, coordinating interdisciplinary assessments and treatment plans, community referrals, and necessary skill-building support.

DOCUMENTATION AND RECORD KEEPING

  • Maintain accurate clinical documentation, ensuring timely updates in medical records and compliance with regulatory standards related to social work services.

QUALIFICATIONS

  • Bachelor's Degree in Social Work or a related field is required.
  • Master's Degree in Social Work is mandatory.

EXPERIENCE

  • A minimum of one year of experience in a healthcare environment, working directly with individuals.

CERTIFICATIONS

  • LSW certification is preferred.
