Position Overview
As a Patient Care Coordinator, you will ensure and facilitate patient care in alignment with the Registered Nurse job description. Collaborating with the Director, you will take responsibility for managing and directing patient care within your designated area. You will also assist in the management of resources, including material, financial, and human assets within the assigned unit.

Key Responsibilities

  • Oversee administrative and operational matters for the assigned care delivery team.
  • Act as a departmental leader to support unit and organizational objectives.
  • Utilize effective communication techniques with patients, families, visitors, staff, and professional colleagues.
  • Promote a Culture of Safety by adhering to established policies and procedures, including timely reporting of patient safety events.
  • Engage in coaching, counseling, and mentoring of unit personnel.
  • Participate in the performance appraisal process for team members.
  • Support the Professional Practice Model and engage in initiatives related to its key components.
  • Assume clinical and administrative responsibilities in the absence of the Director, with appropriate approvals.
  • Ensure patient care is delivered in accordance with all responsibilities outlined in the RN job description.
  • Manage staffing to meet patient care requirements and operational standards.
  • Encourage accountability among staff for compliance with policies, procedures, and professional practices.
  • Ensure timely and fair completion of the unit's scheduling.
  • Apply evidence-based research findings in patient care delivery, share insights with colleagues, and participate in ongoing research initiatives.
  • Provide clinical, consultative, technical, or informational support as needed to staff, other disciplines, patients, and families.

Qualifications

  • Bachelor's Degree in Nursing.
  • A minimum of 3 years of experience as a Registered Nurse in an acute care hospital environment.
  • Ability to swiftly learn and adapt to new technologies, processes, and concepts.
  • Proficient in teaching and guiding others with positivity and encouragement.
  • Demonstrated capability in directing and mentoring personnel.
  • Strong analytical, critical thinking, and problem-solving abilities.
  • Certification in Basic Life Support from the American Heart Association - required upon hire.
  • Current RN licensure in the state of Pennsylvania - required upon hire.

Physical Requirements
Ability to lift and carry up to 25 lbs, with frequent sitting and standing. Frequent use of a keyboard is required. Patient care providers may need to perform role-specific activities, including kneeling, bending, squatting, and performing CPR.
