Patient Care Coordinator

Job Overview:
This role is dedicated to delivering expert nursing care services in alignment with medical directives. Position Summary:
The position employs case management methodologies to guarantee that patients receive thorough and well-coordinated care. Responsibilities include conducting clinical evaluations, formulating patient-focused care strategies, and motivating patients through effective communication techniques. The aim is to elevate care quality and health results for vulnerable populations.
Qualifications:
- Completion of an accredited nursing program
- Active Registered Nursing (RN) license
- A minimum of three (3) years of clinical experience in a hospital environment
- Possession of a valid driver's license
Preferred Qualifications:
- Bachelor's degree in nursing or related field
- Five (5) years of experience in acute, subacute, or home healthcare
- Certification in Case Management
- Bilingual capabilities
Key Responsibilities:
- Prepare for patient engagements
- Oversee all facets of patient care
- Design patient-centered care plans
- Serve as a primary member of patient care teams
- Conduct patient assessments
- Perform home or site visits as necessary
- Communicate effectively with healthcare providers and team members
- Engage in quality improvement initiatives

- Sedentary role with occasional movement required
- Ability to speak, hear, and communicate effectively
- Visual acuity to read computer screens
- Occasional lifting or moving of items up to 25 pounds
