With a workforce of approximately 700 employees operating from multiple offices across a ten-county region, ACRC is one of 21 Regional Centers in California.

COMPENSATION

Compensation varies based on experience and education.

  • Service Coordinator I: $26.78/hr.
  • Service Coordinator IIIB: $31.43/hr. (requires a master's degree and 1 year of social work experience)
  • Service Coordinator IIIA: $34.71/hr. (requires a master's degree and 2 years of post-master's experience)

The actual salary offered will depend on various factors including skills, education, experience, and other relevant job-related elements. It is uncommon for new hires to start at or near the top of the salary range for this role.

POSITION SUMMARY

ACRC is looking for a Service Coordinator to join the Intensive Case Management unit in its Sacramento office. This role involves providing comprehensive case management, community deflection, and advocacy for individuals with developmental disabilities. The position will maintain a maximum caseload ratio of 1:45 in accordance with legislative requirements, focusing on delivering intensive case management to high-acuity clients within the I/DD community. Additionally, this role includes minor Facility Liaison responsibilities, which are crucial for ensuring the health and safety of clients residing in various care facilities.

Key responsibilities include:

  • Monitoring compliance with Title 17 and 22 regulations.
  • Collaborating with Service Coordinators for quality assurance monitoring.
  • Conducting investigations and providing technical support to enhance service quality.
  • Assisting clients and their support networks in accessing and maintaining necessary services.
  • Advocating for clients with community agencies and the criminal justice system.
  • Providing consultation to prevent unnecessary admissions to State Developmental Centers.
  • Tracking and analyzing relevant data trends.
  • Completing all required documentation in line with regulations and ACRC policies.
  • Supporting colleagues with special projects and providing emergency on-call services as needed.
This position serves the Sacramento County area and may require travel throughout California.
