DESCRIPTION

Dwyer Workforce Development (DWD) aims to deliver holistic support to individuals aspiring to thrive in the healthcare field, thereby alleviating workforce shortages and enhancing the quality of life for seniors.

Position Summary: The Case Manager will be responsible for providing comprehensive case management services to Dwyer Scholars.

Position Responsibilities:

1. Develop a thorough understanding of the agency's mission, vision, and values.

2. Engage with applicants to gather necessary information for determining eligibility in accordance with DWD policies.

3. Maintain consistent follow-up with participants through various communication methods.

4. Document service interactions meticulously, including demographic data for evaluation purposes.

5. Ensure the confidentiality of client records and maintain up-to-date tracking systems for participant progress.

6. Collaborate with Dwyer Scholars to identify and address obstacles to their success, utilizing resource solutions and action plans.

7. Provide direct support to Dwyer Scholars in crisis situations, offering guidance and referrals as needed.

8. Facilitate job development activities to secure employment opportunities for Dwyer Scholars.

9. Assist in enhancing job-seeking and workplace readiness skills among Dwyer Scholars.

10. Offer ongoing support to promote job retention and independence for Dwyer Scholars.

11. Uphold client confidentiality and adhere to the Data Privacy Act.

12. Foster teamwork by collaborating with colleagues and participating in required professional development activities.

13. Manage confidential information responsibly and maintain organizational efficiency.

Essential Qualifications:

1. Familiarity with local community resources and social services.

2. Bachelor’s degree in social work, human services, or a related field, or equivalent experience.

3. Strong communication and leadership skills.

4. Proactive attitude with a commitment to learning and contributing.

5. Excellent time-management and organizational skills.

6. Proficiency in office software and willingness to learn new systems.

7. Strong analytical and critical thinking abilities.

8. Knowledge of CRM systems such as Salesforce is a plus.

9. Flexibility to work varied hours as needed.

10. Ability to build effective relationships with diverse populations.

11. Bilingual candidates are preferred.

License/Certification: A valid driver's license and access to a vehicle are required.
