DESCRIPTION

The mission of Dwyer Workforce Development (DWD) is to offer extensive support to individuals striving for a career in healthcare, while also tackling significant workforce challenges and improving senior care.

Position Overview: Execute case management responsibilities and coordinate a wide range of support services for Dwyer Scholars.

Key Responsibilities:

Acquire a thorough understanding of the agency's mission, vision, and values.

Engage with potential candidates and gather necessary information to assess eligibility according to DWD guidelines and required documentation.

Maintain ongoing communication with participants through various channels, including phone, email, and in-person interactions.

Accurately document all service interactions with Dwyer Scholars, Trainers, and Provider/Employer Partners, along with demographic data for evaluation and improvement purposes.

Ensure up-to-date and confidential record-keeping, including tracking the progress of Dwyer Scholars in achieving their training and employment objectives.

Collaborate with Dwyer Scholars to identify obstacles to success and implement self-sufficiency strategies and action plans to overcome these challenges.

Provide immediate assistance to Dwyer Scholars facing crises, both on-site and remotely, including supportive listening, community referrals, goal-setting, and follow-up.

Facilitate job opportunities for Dwyer Scholars through proactive job development and effective communication with Provider/Employer Partners.

Assist Dwyer Scholars in enhancing their job-seeking and workplace readiness skills to prepare for successful employment.

Offer follow-up services to promote job retention and ongoing engagement, supporting Dwyer Scholars in achieving life balance.

Maintain client confidentiality and adhere to the Data Privacy Act.

Exhibit teamwork by collaborating with colleagues as needed. Attend necessary meetings, training sessions, and conferences to enhance expertise and professionalism as scheduled by the Director or funding sources.

Handle confidential information responsibly, demonstrating effective organizational skills and the ability to work independently.

Perform additional duties as assigned.

Essential Qualifications:

Familiarity with local community resources and social services in the relevant geographic area.

A Bachelor’s degree in social work, human services, or a related field, or equivalent experience. Previous experience in human service agencies/programs is preferred.

Strong verbal and written communication skills, along with leadership and motivational abilities.

Proactive approach with a willingness to contribute and learn.

Excellent time-management skills and the capacity to organize and manage multiple projects.

Proficiency in office productivity software and a readiness to learn new systems.

Effective organizational development and critical thinking capabilities.

Familiarity with Salesforce or other CRM systems.

Ability to work flexible hours, including evenings when necessary.

Capacity to establish and maintain effective working relationships with a diverse population.

Bilingual skills are preferred.

License/Certification: A valid driver’s license and access to a vehicle are required.
