DESCRIPTION

The mission of Dwyer Workforce Development (DWD) is to deliver holistic support to individuals striving for a career in healthcare, alleviating workforce shortages, and enhancing senior care.

Position Summary: The Case Manager will perform case management responsibilities and coordinate extensive support services for Dwyer Scholars.

Position Responsibilities:

Acquire a thorough understanding of the agency's mission, vision, and values.

Engage with applicants to gather information for assessing program eligibility in accordance with DWD policies and required documentation.

Maintain regular follow-up communication with participants via various channels including phone, email, and in-person meetings.

Document service interactions meticulously with Dwyer Scholars, Trainers, and Provider/Employer Partners, including demographic data for program evaluation.

Ensure accurate and confidential record-keeping, updating contact notes, and tracking the progress of Dwyer Scholars towards their training and employment objectives.

Collaborate with Dwyer Scholars to identify obstacles to success and implement resource solutions and action plans to overcome these challenges.

Provide direct assistance to Dwyer Scholars in crisis situations, offering supportive listening, community referrals, goal-setting, and follow-up.

Facilitate job opportunities for Dwyer Scholars through proactive job development and effective communication with Provider/Employer Partners.

Assist Dwyer Scholars in enhancing their job-seeking and workplace readiness skills to prepare for successful employment.

Deliver follow-up services to promote job retention and ongoing engagement, helping Dwyer Scholars maintain a balanced life.

Uphold client confidentiality and adhere to the Data Privacy Act.

Exhibit teamwork by collaborating with colleagues as necessary. Attend mandatory meetings, training sessions, and conferences to enhance professional development as scheduled by the Director or funding sources.

Demonstrate responsibility in handling confidential information, organizing tasks effectively, and working independently.

Perform additional duties as assigned.

Essential Qualifications:

Familiarity with local community resources and social services relevant to the geographic area.

A Bachelor’s degree in social work, human services, or a related field, or equivalent experience. Previous experience in human service agencies/programs is preferred.

Strong verbal and written communication skills, along with leadership and motivational abilities.

Proactive attitude with a willingness to contribute and learn.

Excellent time-management skills and the ability to coordinate multiple projects.

Proficiency in office productivity software and a capacity for learning new systems.

Effective organizational development and critical thinking capabilities.

Familiarity with Salesforce or other CRM systems.

Flexibility to work varied hours, including evenings when necessary.

Ability to establish and maintain effective working relationships with a diverse population.

Bilingual skills are preferred.

License/Certification: A valid driver's license and access to a vehicle are required.
