DESCRIPTION

The mission of Dwyer Workforce Development (DWD) is to offer extensive support to individuals seeking career opportunities in the healthcare field, while also tackling the pressing workforce shortage and improving senior care.

Position Summary: The Case Manager will perform case management functions and coordinate a range of support services for Dwyer Scholars.

Position Responsibilities:

Acquire a thorough understanding of the agency's mission, vision, and values.

Engage with applicants to gather necessary information for assessing eligibility in accordance with DWD policies and required documentation.

Maintain ongoing communication with participants through various channels, including phone, email, and in-person meetings.

Document service interactions with Dwyer Scholars, Trainers, and Provider/Employer Partners, ensuring accuracy and thoroughness in all records.

Manage confidential record-keeping systems, including updating contact notes and tracking the progress of Dwyer Scholars in achieving their training and employment objectives.

Collaborate with Dwyer Scholars to identify obstacles to success and utilize self-sufficiency development tools and action plans to address these challenges.

Provide immediate assistance to Dwyer Scholars facing crises, offering supportive listening, community referrals, goal-setting, and appropriate follow-up.

Facilitate job placement opportunities for Dwyer Scholars through proactive job development efforts and effective communication with Provider/Employer Partners.

Assist Dwyer Scholars in refining their job-seeking and workplace readiness skills to prepare for successful employment.

Deliver follow-up services to promote job retention and ongoing engagement, supporting Dwyer Scholars in achieving a balanced life.

Uphold client confidentiality and adhere to the Data Privacy Act.

Exhibit teamwork by collaborating with colleagues as needed, attending required meetings, training sessions, and professional development opportunities to enhance expertise and professionalism.

Must demonstrate responsibility in handling confidential information, possess strong organizational skills, and work independently.

Other duties as assigned.

Essential Qualifications:

Familiarity with local community resources and social services in the relevant geographic area.

Bachelor's degree in social work, human services, or a related field, or equivalent experience. Previous experience in human services is preferred.

Strong verbal and written communication skills, along with leadership and motivational abilities.

Proactive attitude with a willingness to contribute and learn.

Excellent time-management skills and the ability to coordinate multiple projects effectively.

Proficiency in office productivity software and a capacity for learning new systems.

Strong organizational development and critical thinking skills.

Familiarity with Salesforce or other CRM systems.

Ability to work flexible hours, including evenings when necessary.

Capability to establish and maintain effective working relationships with a diverse population.

Bilingual skills are preferred.

License/Certification: A valid driver's license and access to a vehicle are required.
