Veteran Support Specialist

This role is integral to the United States Veteran Initiative, reporting directly to the Program Coordinator or Clinical Director. The Veteran Support Specialist is tasked with empowering veterans to attain their utmost level of self-sufficiency. This position involves a comprehensive case management approach that includes assessment, planning, implementation, coordination, monitoring, and evaluation of services tailored to meet the unique needs of each veteran.

Key Responsibilities:
1. Conduct thorough assessments of veterans referred to the program, identifying challenges and needs across various domains, including:
- Housing
- Employment
- Education/Training
- Financial Stability
- Sobriety Support/Addiction Treatment
- Legal Matters
- Family/Social Relationships
- Medical/Psychiatric Support
2. Collaborate with VA personnel and community partners to facilitate eligibility documentation and access to mental and physical health services.
3. Oversee the client intake process, ensuring that all necessary eligibility documentation is accurately compiled and maintained.
4. Work alongside clients to develop Individual Action Plans (IAPs) that outline their goals and strategies for success.
5. Provide mentorship and guidance to junior Case Managers regarding client IAPs, intervention strategies, and documentation practices during regular program meetings.
6. Engage in crisis intervention as needed, supporting junior staff with client-related challenges.
7. Maintain confidentiality of client files, ensuring compliance with organizational standards and the most current documentation practices.
8. Schedule and conduct regular meetings with clients, documenting progress using the Data/Assessment/Plan (DAP) format.
9. Review and update IAPs on a monthly basis, ensuring they reflect the current status and needs of clients.
10. Prepare timely discharge summaries for clients transitioning out of the program.
11. Conduct home visits to provide essential case management services and monitor the well-being of clients.
12. Participate in quality management initiatives, data collection, and report preparation.
13. Facilitate life skills classes to enhance client capabilities.
14. Offer specialized case management services and life skills training tailored to specific veteran subpopulations, such as female veterans or those with disabilities.
15. Perform random drug and alcohol screenings to ensure compliance and support recovery efforts.
16. Uphold confidentiality standards in accordance with the Health Insurance Portability and Accountability Act (HIPAA).
