Key Responsibilities:

- Advocacy: Represent veterans' needs and facilitate access to necessary services.
- Team Meetings: Participate in regular meetings and case conferences to discuss and strategize on veteran care.
- Direct Support: Manage a caseload of 25 veterans, providing clinical case management and direct support.
- Housing Navigation: Assist veterans in understanding and navigating the Housing Authority voucher process to secure permanent supportive housing.
- Life Skills Education: Offer guidance on community living and essential life skills, including financial management and personal safety.
- Resource Linkage: Connect veterans with VA medical facilities, community agencies, and various services for health care, benefits, and vocational assistance.
- Mental Health Support: Provide mental health and substance abuse counseling, utilizing the Harm Reduction Model, or refer to appropriate providers.
- Crisis Management: Deliver crisis intervention services as needed.
- Documentation: Involve veterans in creating interdisciplinary treatment plans and maintain accurate records in compliance with VA standards.
Required Skills:
Ideal candidates will possess knowledge of DSM diagnoses, assessment techniques, and evidence-based practices such as Motivational Interviewing and Trauma-Informed Care. Strong collaboration skills and the ability to manage time effectively are essential.
Qualifications:
Applicants should have a minimum of a bachelor's degree in social sciences or family therapy, with a master's degree preferred. A minimum of five years of case management experience with high-need populations is required. Candidates with a master's degree need only one year of relevant experience. A valid driver's license and reliable transportation are necessary for this role.
Work Environment:
This position involves working directly with individuals experiencing homelessness, which may present various challenges. Staff will receive training to handle crisis situations effectively and ensure a safe working environment.
