Job Description

We are seeking an experienced Registered Nurse (RN) to join our Hospice Team as an RN Hospice Coordinator. In this vital role, you will oversee the coordination and implementation of hospice care services, ensuring seamless transitions and high-quality patient outcomes.

  • Collaborate with interdisciplinary teams to develop and implement comprehensive plans of care.
  • Coordinate referrals, admissions, and discharges, ensuring timely and efficient care delivery.
  • Maintain accurate and up-to-date records, adhering to regulatory requirements and organizational standards.
  • Provide education and support to patients, families, and healthcare providers to promote informed decision-making and effective care planning.

Requirements:

  • Current RN licensure in the state of Florida.
  • Minimum 1 year of experience in medical-surgical nursing or equivalent.
  • Preferred experience in hospice care or palliative care.
  • Certification in Hospice/Palliative care is an asset.
