Nursing Home Health Clinical Director

About This Role:
We're seeking an experienced RN Clinical Manager for our Home Health team. As a key member of our leadership team, you'll be responsible for overall quality of patient care and clinical performance of nurses and aides. Your responsibilities will include:
- Recommending employment decisions for staff to the director of operations
- Organizing clinical operations for the care center
- Reviewing requests for services and determining patient eligibility/suitability for home care services
- Overseeing scheduling and payor verification/precertification requirements
- Ensuring accurate documentation from staff members
- Assisting clinicians in establishing patient goals and developing Plans of Care (POC)
- Participating in the on-call process and resolving patient/family/physician concerns
Requirements:
To succeed in this role, you'll need:
- An Associate or Bachelor's degree in nursing
- A current RN license specific to the state(s) you're assigned to work
- At least one year of clinical RN experience in home health or hospice
- One year of RN management experience
- Current CPR certification
