Job Description

We are looking for a dedicated Clinical Care Specialist to work in our Admissions Department. The successful candidate will be responsible for coordinating and implementing the overall plan of care for patients in various hospice settings, ensuring they receive the best possible quality of life.

Responsibilities:
  • Provide all aspects of care for terminally ill patients
  • Deliver patient and family education
  • Complete initial and ongoing assessments
  • Manage pain and symptoms effectively
  • Develop an interdisciplinary plan of care reflecting patient-centered goals
  • Apply critical thinking skills to ensure quality care
  • Maintain professional relationships with colleagues, physicians, and patients
Requirements

To be eligible for this role, candidates must possess:

  • A valid RN license in the State of Florida
  • An Associate degree from an accredited School of Nursing (Bachelor's degree preferred)
  • A valid driving license and good driving record
  • Excellent interpersonal communication skills
  • Personal and professional development, accountability, organization, and leadership in nursing practice
Benefits and Compensation
