Job DescriptionWe are seeking an experienced Registered Nurse (RN) to join our Hospice Team as a Hospice Care Specialist. In this role, you will provide specialized care and support to terminally ill patients and their families, ensuring a dignified and peaceful end-of-life experience.
- Deliver personalized care and attention to each patient, respecting their unique needs and preferences.
- Develop and implement individualized plans of care in collaboration with the patient, family, and healthcare team.
- Facilitate open communication and education among patients, families, and healthcare providers to ensure informed decision-making.
- Maintain accurate and up-to-date records, adhering to regulatory requirements and organizational standards.
Requirements:
- Current RN licensure in the state of Florida.
- Minimum 1 year of experience in medical-surgical nursing or equivalent.
- Preferred experience in hospice care or palliative care.
- Certification in Hospice/Palliative care is an asset.
