GENERAL JOB DESCRIPTION

The PRS Case Manager (Level III) is required to possess expertise in trauma-informed care, child welfare methodologies, case management, and conducting home evaluations in accordance with state regulations. Proficiency in drafting comprehensive and professional assessment reports is essential. The PRS Case Manager should be well-versed in local social services, community engagement, and have experience working with immigrant communities. This role involves delivering intensive case management services with a focus on family preservation, kinship care, and effective collaboration with local resource providers to facilitate post-release services for children and their sponsors.

Flexibility and adaptability are crucial, along with the capability to assess needs and circumstances accurately. Clear communication, both verbal and written, is necessary, as is the ability to gather, analyze, and interpret information effectively. The candidate should be adept at navigating conflicts sensitively and devising practical solutions while maintaining composure under pressure. A positive attitude in challenging situations, along with strong organizational skills to manage multiple projects in alignment with Urban Strategies' objectives, is essential.

WHAT YOU'LL BE DOING
  • Manage a fluctuating caseload of post-release services (Levels I, II & III).
  • Adhere to US and ORR policies, procedures, and mandated timelines.
  • Conduct interviews with care provider staff, children, and sponsors.
  • Continuously assess safety and identify areas requiring additional support and resources.
  • Maintain regular communication and conduct virtual and in-person home visits.
  • Provide crisis intervention services and connect families to necessary resources.
  • Collaborate with children and sponsors to develop safety plans.
  • Act as a liaison for children, sponsors, and external agencies.
  • Utilize trauma-focused methodologies and therapeutic support for families.
  • Deliver culturally and linguistically appropriate services through comprehensive clinical case management.
  • Ensure proper documentation and confidentiality of records.
  • Report any concerns regarding children and sponsors to the supervisor and ORR.
  • Participate in training and quality improvement initiatives.
  • This role requires travel for in-person home visits across various locations.
  • Perform related duties as assigned, adapting to program needs.
MINIMUM EXPERIENCE and/or REQUIREMENTS TO BE CONSIDERED
  • A Master of Social Work (MSW) or equivalent degree in psychology, sociology, or a related field; or a bachelor's degree with at least 5 years of clinical experience.
  • Proficient in writing detailed assessment reports and experienced in working with immigrant populations.
  • Knowledgeable in family preservation, kinship care, and trauma-informed practices.
  • Experience in providing Post Release Services and engaging with immigrant communities.
  • Strong communication and interpersonal skills for effective interaction with families and stakeholders.
  • Ability to manage sensitive information with confidentiality.
  • Fluent in English and Spanish (verbal/written) and proficient in Microsoft Office.
  • A valid driver's license and willingness to travel for home visits.
