ROLE OVERVIEW
The Advanced Case Manager is responsible for delivering comprehensive case management services, particularly focusing on trauma-informed care and child welfare practices. This role demands expertise in managing cases and conducting home assessments in compliance with state regulations. The ideal candidate will possess strong skills in drafting detailed assessment reports and will have a deep understanding of local social services and community outreach, especially in relation to immigrant populations.
KEY RESPONSIBILITIES
- Manage a fluctuating caseload of post-release services.
- Adhere to US and ORR policies, ensuring compliance with mandated timelines.
- Conduct interviews with care providers, children, and sponsors.
- Continuously assess safety and identify additional support needs for children.
- Maintain regular contact through virtual and in-person visits.
- Provide crisis intervention and connect families to necessary services.
- Collaborate with families and external agencies to ensure child safety.
- Utilize trauma-informed approaches to support children and families.
- Document all interactions and maintain confidentiality in line with program regulations.
- Report any concerns regarding child welfare to the appropriate authorities.
- Participate in ongoing training and quality improvement initiatives.
- Travel as required for home visits across various locations.
- Master's degree in Social Work (MSW) or a related field, or a Bachelor's degree with significant clinical experience.
- Proven ability to write comprehensive assessment reports.
- Experience in trauma-informed practices and child welfare policies.
- Strong communication skills for effective interaction with families and stakeholders.
- Fluency in English and Spanish, both verbal and written.
- Valid driver's license and willingness to travel for home visits.
