Patient Services Coordinator

The Patient Services Coordinator plays a vital role in assisting the Social Worker by performing specific tasks to facilitate throughput and discharge.
This position serves as a liaison for patient care activities for the Social Work department, working to provide and improve continuity to the patient's experience.
Responsibilities:- Assists patients/care partners with concrete needs, including transportation, uncomplicated DME, medication pickup and delivery, and parking passes.
- Facilitates tasks related to discharge, including distribution of MetroCard, petty cash, and patient clothing from the Social Work central office.
- Works cohesively with Social Worker to obtain community resources and support services, such as Meals on Wheels and food pantries.
- Collaborates with Social Worker to resolve issues impacting patients' care progression and assists with resumption of home care services for MLTC patients.
- Monitors and updates the medical record in accordance with standards set for the Department of Social Work and/or regulatory agencies.
- Assists with referrals to REAP through Epic and NYLAG, following established protocols.
- Provides front-end coverage as needed and performs specific assignments relevant to particular programs.
- Provides recommendations for process/service improvement to promote patient satisfaction and works with Social Worker to address issues.
