About the Role

The Patient Services Coordinator plays a vital role in the delivery of high-quality healthcare services at the Mount Sinai Health System. As a key member of our team, you will serve as a liaison for patient care activities, ensuring seamless coordination and communication between patients, healthcare providers, and administrative staff.

Key Responsibilities
  • Facilitate Patient Care Plans: Collaborate with Primary Care Providers to develop and implement patient care plans, participating in daily huddles and monthly case discussions to ensure effective care coordination.
  • Coordinate Clinical Activities: Work closely with clinical staff to resolve issues impacting patient care progression, ensuring timely and efficient delivery of services.
  • Monitor and Update Patient Information: Utilize dashboards and reminders to stay organized and ensure accurate and up-to-date patient information, including visit status, follow-up appointments, and authorization requirements.
  • Communicate with Insurance Carriers: Contact insurance carriers to obtain pre-certification and resolve billing errors, working closely with the Department/Practice Manager to address patient needs and escalated issues.
  • Provide Front-End Coverage: Offer support as needed, recording telephone encounters and routing them to the appropriate providers.
  • Connect Patients with Resources: Serve as a liaison between patients, physicians, and clinical staff, connecting patients with relevant internal and external resources to promote patient satisfaction and well-being.
  • Contribute to Process Improvement: Provide recommendations for process improvements to enhance patient satisfaction and address issues, promoting a culture of continuous quality improvement.
Requirements
  • Education: Some college; Bachelor's degree preferred
  • Experience: 3 years of healthcare/customer service or relevant experience
