Job Description

The Patient Services Coordinator plays a vital role in delivering high-quality patient care at Mount Sinai Health System. As a liaison between patients, physicians, and clinical staff, you will ensure seamless coordination of care and provide exceptional support to patients and families.

Key Responsibilities:
  • Collaborate with Primary Care Providers to facilitate patient care plans, participating in daily huddles and monthly case discussions.
  • Work closely with clinical staff to resolve issues impacting patient care progression.
  • Monitor and update dashboards with reminders and alerts to organize daily tasks, including visit status reminders, follow-up appointments, and authorization requests.
  • Contact insurance carriers for pre-certification and consult with Department/Practice Managers to address patient needs or escalated issues.
  • Provide front-end coverage as needed, recording telephone encounters for routing to appropriate Providers.
  • Act as a liaison with Physicians, patients, and clinical staff to connect patients with relevant internal and external resources.
  • Recommend process and service improvements to promote patient satisfaction and address issues.
Qualifications:

Some college; Bachelor's degree preferred. 3 years of healthcare/customer service or relevant experience.
