We are seeking a skilled LPN Intake Coordinator to join our Patient Care team. As a key member of our intake team, you will be responsible for receiving and analyzing incoming referrals and orders, ensuring seamless communication with our clinical staff and external partners.
Key Responsibilities:- Receive and analyze incoming referrals and orders, ensuring accurate and timely processing.
- Communicate effectively with patients, families, physicians, and hospitals regarding home health services available.
- Coordinate communication of referral information with Intake Specialist RN and other clinical staff.
- Verify new referring physician licenses and research coverage or pay source for each referral.
- Obtain authorization for visits from insurance or Medicaid as appropriate.
- Manage patient ping feed throughout the day and disseminate patient ping data to relevant staff members.
- Current LPN licensure in state of practice.
- Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
- CPR Certification required.
- Minimum of one (1) year experience in medical office procedures/intake.
