We are seeking a highly skilled Patient Services Coordinator to join our team at Mount Sinai Health System. As a key member of our healthcare team, you will play a vital role in ensuring the delivery of exceptional patient care.
Key Responsibilities- Collaborate with Primary Care Providers to facilitate patient care plans, participating in daily huddles and monthly case discussions.
- Coordinate appointments, information, and continuing care requirements to ensure seamless patient care.
- Monitor and update dashboards with reminders and alerts to organize daily tasks, including visit status reminders, follow-up appointments, and authorization for medical review.
- Communicate with insurance carriers for pre-certification and consult with Department/Practice Manager to address patient needs or escalated issues.
- Provide front-end coverage as needed, recording telephone encounters for routing to appropriate Providers.
- Work as a liaison with Physicians, patients, and clinical staff to connect patients with relevant internal/external resources.
- Recommend process/service improvements to promote patient satisfaction and address issues.
